PERFORMANCE EVALUATION FORMS

PERFORMANCE EVALUATION/GOAL SETTING INFORMATION & INSTRUCTIONS

The performance evaluation process starts with the employee. You, as the employee, should be thinking about goals you want to achieve for the year and what information, tools, or resources you need to achieve those goals.

During the end of year wrap up and new year goal setting meeting (Dec/Jan), you should bring your ideas forward and any comments you have about your accomplishments. This meeting is the opportunity for your manager to review your performance, job description, give you feedback, and set goals and a plan to achieve those goals for the coming year. Your goals should align with the department and City goals as a whole.

In the June/July time-frame, your manager should be conducting a mid-year evaluation with you as the employee. You should be discussing the progress being made on your goals, and decide if any adjustments to the goals should be made.

If you have any questions about the process, please contact your supervisor or Human Resources.
